This form is ONLY for University of Toronto undergraduate students who wish to submit a formal petition request to the Department of Anthropology at the University of Toronto Mississauga (UTM). 

This form is NOT for types of petitions handled by the UTM Office of the Registrar.

If your request is about missing pre-requisites for a UTM Anthropology course, there must be space in the course during the first week of classes. Check the Timetable during the first week of classes BEFORE you submit this form to see if there is space available in the course.
If your request is to reconsider program entry (after being refused on ACORN), select the program you wish to enrol in:

Please clearly state the strong and valid reason(s) in support of your request. This box is resizable.
For example, if you are missing ANT202H5, then mention another course code (e.g. a biology course, or similar) that may show that you could still do well in the course.

IMPORTANT: Please read before you submit this request:

  • It is my responsibility to check my utoronto email account regularly.
  • The decision will be sent to my utoronto email account. (Please allow two business days (on average) to hear from the Undergraduate Advisor.)
  • If a Department of Anthropology faculty or staff member has asked you to submit supporting documentation along with your formal petition, please send the attachments via email to the Academic Advisor & Undergraduate Program Administrator at utm.anthro@utoronto.ca.
  • If more information or documentation is be required to support my request this may delay a final decision.
  • The Academic Advisor & Undergraduate Program Administrator consults with the Chair and instructor as required.
  • All decisions made by the Chair are final.
  • Requests without strong and valid reasons will not be considered.
  • This form is NOT for types of petitions handled by the UTM Office of the Registrar.
