This form helps us understand your goals and support needs so we can match you with a peer mentor. The information is reviewed by PSP Program staff (Registered Nurses at the Health & Counselling Centre) and will be kept confidential. Please note that an exact match may not always be possible.
These questions help us understand what you're looking for in a peer mentor and whether the PSP program is the right fit for your needs. While mentors may provide some general academic guidance and share strategies for navigating university life, the primary focus of the PSP program is to support the development of healthy wellness habits and overall well-being.
This section helps us determine the appropriateness of PSP peer support and whether additional care may be needed.
At your first and every subsequent meeting, your mentor will complete a brief mental health screening with you. If the screening suggests that more support is needed, your mentor will help you connect with appropriate services.
