Documentation Requirements for LD/ADHD

Please review the information below regarding documentation requirements for ADHD and Learning Disabilities:

Attention Deficit / Hyperactivity Disorder (ADHD)

  • A copy of a recent psychoeducational report (no older than five [5] years old or completed at age eighteen or later)

    and/or
     
  • UTM Certificate of Disability including the criteria for ADHD, completed by a health care practitioner who is familiar with the disability; your disability falls within their scope of practice and who is licensed to make a diagnosis. Documentation must indicate adult functional impacts of ADHD and recommended accommodations/strategies. As such, updated documentation may be necessary.
     
  • If applicable, a copy of the most recent Individual Education Plan (IEP), or letter from your high school citing the accommodations received.
  • If applicable, a copy of the most recent ADHD Checklist/Screeners administered.
  • Note: IEPs and ADHD Checklists/Screeners can be submitted as additional documentation, but do not suffice on their own for full documentation or full accommodations.
     

Learning Disability

  • Copy of a comprehensive psychoeducational report (no older than five [5] years old or completed at age eighteen or later) stating the nature of the learning disability, all test scores, a description of the academic difficulties resulting from the disability, and recommended accommodations.
  • An Individual Education Plan or letter from your high school citing the accommodations received (if applicable).
