Two useful forms used in our department can be found by clicking on the links below:

Verification of Student Illness or Injury form
- Review the information on the Office of the Registrar's website to understand when this form is needed for petitions. Refer to your course syllabus to determine the policy for missed tests or assignments.
Verification of Extenuating Circumstances form
ACORN Absence Declaration

We normally don't allow students to join a course without having the required prerequisite(s) (except for some first year courses). Joining a course without a prerequisite means that you probably won't have the necessary background and knowledge needed to complete the course successfully.

However, if you feel that you do have the required background (e.g., from a course taken at another institution), you can request a waiver by completing an online Prerequisite Waiver Request Form at the time you have enrolled (or waitlisted) in the course and by no later than the first day of classes. Please see the links below to find the specific form for the subject area. Kindly submit one form per course. You will need to submit the form using your @mail.utoronto.ca email address.

For MAT & STA waivers, students will only receive an email response if their request is denied.

Summer 2025

Submission Deadline: The last day to add S courses for Summer 2025 is July 8, 2025. The deadline to submit a prerequisite waiver form for a Summer S course is July 4, 2025 at 4:00 pm.

Fall 2025

Submission Deadline: Waiver forms will open on June 25, 2025 at 8:00 a.m.*** The last day to add F/Y courses is September 15, 2025. The deadline to submit a waiver for an F or Y course is September 12, 2025 at 12:00 pm.

The submission will be reviewed by the respective Associate Chair. Note that there is no guarantee of a successful outcome.

IMPORTANT NOTE: Courses with only high school prerequisites (like MAT102H5, first-year calculus courses, or MAT223H5) do not require a prerequisite waiver form. We do not check high school prerequisites for these courses.
