Refund Request Form

To initiate an eCommerce refund, departments are required to complete the eCommerce Refund Form available on the UTM Budget, Planning & Finance website. Before submitting, verify the form is complete and accurate—ensure all required fields are filled in, and totals add up correctly. Incomplete or incorrect forms may delay processing
