Please review our Rental Policies and Procedure page prior to completing this form to see what is required to request a rental of the facilities of the UTM Recreation, Athletics and Wellness Centre. 

IMPORTANT! If you are a recognized UTM Student Club/Group looking to rent our facilities, please use the Student Group Rental Form and follow the steps prescribed in the page.

Please factor in tear-down
Indicate if you have a preference on day of week (rank order your 1st, 2nd, 3rd choices), ideal number of bookings and any other details outside of the days/times listed above. It is also helpful to include exception dates to your booking request.
