Description of Duties & Allocation of Hours Form
The Description of Duties & Allocation of Hours form indicates how your TA hours will be allocated for a course. TAs must sign and date this form twice: once at the start of the term and once at the mid-term review of hours. The signed and dated form must be returned to the course instructor and Departmental Secretary both times.
If you have any concerns about running out of TA hours, please contact the course instructor.
Shuttle Bus reimbursement forms and procedures for tickets purchased for the Fall 2016 term are on the Forms for TAs page.
